About The Position

Must-Have :
Description :
30 Bed unit with an average of 100-110 patients daily and up to 120 patients during winter season.
Perform patient assessments according to unit policy.
Conduct initial assessments based on the patient's clinical presentation, care setting, age, development level, and cultural orientation in a timely manner.
Assess physical, psychological, and social status, including pain, risk for falls, cognitive skills, and other patient needs.
Identify and prioritize the need for further assessment based on the patient’s condition and responses to care.
Initiate discharge planning upon admission as appropriate.
Develop a Patient Plan of Care tailored to each patient based on their assessed needs, incorporating input from the patient, family, and multi-disciplinary health care team members.
Identify specific nursing diagnoses relevant to assessment findings.
Establish interventions for each nursing diagnosis based on identified patient needs.
Implement interventions identified in the Interdisciplinary Plan of Care.
Effectively communicate patient care needs to all members of the multidisciplinary health care team.
Conduct patient and family education that is suitable for the patient's age, learning needs, and readiness to learn, including family members when appropriate.
Document nursing interventions and patient education on designated forms.
Evaluate and document the patient’s response to the Plan of Care and interventions.
Revise and update the Plan of Care as necessary, with appropriate documentation.
Document the patient’s response to education appropriately.
Facilitate processes in response to situations that could negatively impact patient outcomes.
Follow the five rights of medication administration and maintain transcription accuracy.
Administer blood products according to policy.
Administer IV therapy in compliance with hospital policy.
Function effectively during emergency patient care situations.
Implement appropriate interventions related to the use of restraints according to hospital policy.
Perform waived testing in accordance with hospital policy.
Assume responsibility for maintaining a high level of clinical expertise and adapting to new information, technology, and evolving trends in nursing practice.
Maintain required certifications.
Demonstrate effective written and verbal communication skills in English.
Calculate figures and amounts such as proportions, percentages, and volumes.
Define problems, collect data, establish facts, and draw valid conclusions.
Work efficiently in a high volume, fluctuating census environment with frequent interruptions.
