  • Full-time opportunity for a Board-Certified or Board-Eligible Internist or Family Medicine physician with geriatrics experience
  • Provide overall medical direction and supervision for a Program of All-Inclusive Care for the Elderly (PACE)
  • Administrative reporting to the Executive Director; clinical oversight of Primary Care Providers and Advanced Practice Providers
  • Deliver comprehensive care to seniors age 55+ who qualify for nursing home level care but live in the community
  • Conduct initial and periodic medical assessments, develop individualized care plans, and provide direct patient care as needed
  • Participate in intake and assessment of new applicants; coordinate closely with caregivers and the interdisciplinary team
  • Discuss diagnoses, prognosis, and care options with participants and families; help participants plan future care based on their wishes
  • Collaborate on nursing and interdisciplinary treatment plans and care conferences
  • Oversee and maintain medical policies, procedures, and clinical protocols; lead quality assurance and performance improvement initiatives
  • Educate patients, families, and staff regarding chronic disease management and health maintenance
  • Supervise the care provided by the interdisciplinary team; participate in provider on-call rotation as required
  • MD or DO with unrestricted (or eligible) state medical license; experience in geriatric care or PACE program preferred
  • Competitive base salary in the $240,000'$270,000 range plus performance incentives and comprehensive benefits
