• Call : 1 : 5 telephone only, and supported by nurse advisory line
• Model : Patient Center Medical Home Model

• Schedule : Mon - Fri with flexible scheduling
• Job Description : Typical outpatient Primary Care services
• Patient Population : All Ages or Adult Medicine
• Patient Load : 18-22 PPD
• EMR : Epic
• Support : 2 experienced APPs Full admin staff
