Includes rotating every 4th weekend and holiday.
EverHeart Hospice is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to deliver high-quality, direct patient care during evening and overnight shifts.
Ideal candidates will be team-oriented professionals with a solid clinical background, passionate about providing comfort, dignity, and exceptional care to patients and their families during after-hours.
Responsibilities :
Provide competent nursing care to assigned patient through skilled symptom management, emotional support, appropriate teaching, and timely response to patient and family needs, including :
- Routine home care visits.
- End-of-life / continuous care.
- Referrals.
- Care center assistance.
- Chart reviews.
- Other duties as assigned.
- Recognize and report promptly to the registered nurse any changes in the mental and / or physical status of the patient. Reporting should be written and oral when appropriate.
- Explain hospice services and benefits in regard to Medicare, Medicaid, and private insurance, and know when to use palliative care interventions and ABNs.
- Obtain and enter orders from medical directors and physicians when deemed appropriate.
- Order appropriate Durable Medical Equipment (DME) for the patient.
- Facilitate continuity of care by coordinating admission of home-care patients to inpatient settings (hospital, nursing home), as needed.
- Work evenings and weekends as needed to meet the needs of the patients and families, and the agency backup schedule.
- Complete all documentation as required by the organization in compliance with agency policy, accreditation bodies, and governing requirements.
- Maintain clinical competency for assigned responsibilities and procedures.
- Other duties as assigned.
Qualifications :
Education and Experience :
Graduation from an approved School of Nursing.Current LPN licensure in the State of Ohio.Minimum of two years of experience in Medical / Surgical, Oncology, or community health nursing.Special Skills and Requirements :
Shall have a valid driver’s license.Excellent customer service skills.Detailed with excellent communication, documentation, and organization.Ability to follow instructions.Ability to work independently without supervision.Travel to other sites as needed. Travel may be in inclement weather.Ability to lift 25 lbs of supplies or equipment.Requires various amounts of sitting, standing, twisting, turning, bending, lifting, and walking.Ability to assist with patient care activities such as ambulation, transfers, and positioning.About Company
