Schedule : 4 : 00 PM-12 : 00 AM

Includes rotating every 4th weekend and holiday.

EverHeart Hospice is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to deliver high-quality, direct patient care during evening and overnight shifts.

Ideal candidates will be team-oriented professionals with a solid clinical background, passionate about providing comfort, dignity, and exceptional care to patients and their families during after-hours.

Responsibilities :

Provide competent nursing care to assigned patient through skilled symptom management, emotional support, appropriate teaching, and timely response to patient and family needs, including :

• Routine home care visits.
• End-of-life / continuous care.
• Referrals.
• Care center assistance.
• Chart reviews.
• Other duties as assigned.
• Recognize and report promptly to the registered nurse any changes in the mental and / or physical status of the patient. Reporting should be written and oral when appropriate.
• Explain hospice services and benefits in regard to Medicare, Medicaid, and private insurance, and know when to use palliative care interventions and ABNs.
• Obtain and enter orders from medical directors and physicians when deemed appropriate.
• Order appropriate Durable Medical Equipment (DME) for the patient.
• Facilitate continuity of care by coordinating admission of home-care patients to inpatient settings (hospital, nursing home), as needed.
• Work evenings and weekends as needed to meet the needs of the patients and families, and the agency backup schedule.
• Complete all documentation as required by the organization in compliance with agency policy, accreditation bodies, and governing requirements.
• Maintain clinical competency for assigned responsibilities and procedures.
• Other duties as assigned.
