Nurse Practitioner

What You’ll Do :

Conduct patient assessments, diagnoses, and manage treatment plans

Collaborate with facility staff, physicians, and families on resident care

Monitor and follow up on chronic and acute conditions

Communicate with interdisciplinary teams to ensure continuity of care

Document services provided and outcomes using EMR

What You’ll Need :

Licensed Nurse Practitioner in New York

Experience in skilled nursing or long-term care preferred

Strong clinical assessment and decision-making skills

Excellent communication and teamwork abilities
