Job Overview : As a Case Manager with a background in nursing, you will be responsible for coordinating and managing the care of patients throughout their healthcare journey. You will work closely with multidisciplinary teams, patients, and their families to assess needs, develop care plans, and facilitate effective communication between all stakeholders. The primary goal is to ensure the provision of high-quality, cost-effective healthcare services and promote positive patient experiences.

Key Responsibilities :

  • Assessment and Care Planning :
  • Conduct comprehensive assessments of patients' medical, social, and psychological needs.

  • Collaborate with healthcare providers to develop individualized care plans.
  • Monitor and update care plans as necessary to ensure they align with patients' evolving needs.
  • Coordination of Care :
  • Serve as a liaison between patients, healthcare providers, and other involved parties.

  • Facilitate communication to ensure seamless transitions of care.
  • Coordinate referrals to specialists, ancillary services, and community resources.
  • Advocacy and Education :
  • Advocate for patients' rights and preferences while respecting their autonomy.

  • Provide education to patients and their families regarding their conditions, treatment options, and self-care.
  • Utilization Review and Resource Management :
  • Monitor the appropriate utilization of healthcare resources.

  • Collaborate with insurance providers to ensure coverage for necessary services.
  • Identify opportunities for cost-effective care.
  • Quality Assurance and Documentation :
  • Maintain accurate and thorough documentation of patient assessments, care plans, and interventions.

  • Participate in quality improvement initiatives to enhance patient outcomes.
  • Discharge Planning :
  • Coordinate safe and timely discharges, including arranging for home care or post-acute services.

  • Ensure patients have access to necessary medications and follow-up appointments.
  • Qualifications :

  • Current Registered Nurse (RN) license in state of practice and in good standing.
  • Bachelor of Science in Nursing (BSN) preferred.
  • Previous experience in case management, care coordination, or a related field.
  • Strong clinical assessment and critical thinking skills.
  • Excellent communication and interpersonal skills.
  • Knowledge of healthcare regulations, reimbursement systems, and community resources.
  • Ability to work independently and as part of a collaborative team.
