Job Overview : As a Case Manager with a background in nursing, you will be responsible for coordinating and managing the care of patients throughout their healthcare journey. You will work closely with multidisciplinary teams, patients, and their families to assess needs, develop care plans, and facilitate effective communication between all stakeholders. The primary goal is to ensure the provision of high-quality, cost-effective healthcare services and promote positive patient experiences.

Key Responsibilities :

Conduct comprehensive assessments of patients' medical, social, and psychological needs.

Serve as a liaison between patients, healthcare providers, and other involved parties.

Advocate for patients' rights and preferences while respecting their autonomy.

Monitor the appropriate utilization of healthcare resources.

Maintain accurate and thorough documentation of patient assessments, care plans, and interventions.

Coordinate safe and timely discharges, including arranging for home care or post-acute services.
