What You'll Do

  • Evaluate and develop treatment plans for patients of all ages.
  • Administer physical therapy treatments to restore function and relieve pain.
  • Document patient visits and update treatment goals promptly.
  • Communicate with physicians and coordinate care with other disciplines.
  • Perform comprehensive assessments and recommend home safety improvements.
  • Educate patients and caregivers on home exercise programs.
  • Complete functional assessments and OASIS documentation.
  • Supervise assistants in patient care settings.

What You'll Bring

  • Licensed Physical Therapist (PT) in Illinois.
  • Bachelor's Degree in Physical Therapy.
  • Basic Life Support (BLS) certification within 30 days.
  • Valid driver's license and proof of auto insurance.
  • Strong communication and documentation skills.

    • Salary, Shifts & Location

    The compensation range for this position is approximately $76,000 - $131,000 annually, based on an hourly rate of $36.74 - $63.19. Hours may vary depending on patient census and program needs.
