Pyramid Consulting is seeking a Registered Nurse (RN) Clinical Nurse Coordinator Senior for a nursing job in Alpharetta, Georgia.

The Senior RN Care Coordinator (PRN, Day Shift)

Key Responsibilities

Assess patient medical, psychosocial, and resource needs for discharge readiness.

Collaborate with physicians, utilization review nurses, and interdisciplinary care teams to optimize care progression and reduce delays.

Develop and implement individualized discharge plans, ensuring consensus among patients, families, physicians, and payers.

Identify and resolve barriers to disposition, initiate post-acute referrals, and coordinate with social work as needed.

Document assessments, plans, and services accurately in the medical record.

Precept and mentor new hires and student interns in case management best practices.

Promote evidence-based care, regulatory compliance, and cultural sensitivity in care transitions.

Requirements

Bachelor’s in Nursing required.

Active Georgia RN license (or Compact license) and current BLS certification required.

ACM or CCM certification preferred.

Minimum 3 years acute care RN experience and 2 years case management experience in hospital or payer-based setting.

Strong communication, assessment, problem-solving, and collaboration skills, with knowledge of regulations and community resources.
