Home Care Registered Nurse (RN) - Per Diem

Overview\nTrusting & Loving Care In-Home Services is a Medicaid-approved home care agency providing CDS, In-Home Personal Care, Private Duty Nursing (PDN), and Healthy Children & Youth (HCY) services throughout the Greater St. Louis area. We are expanding our Private Duty Nursing program and seeking a skilled, dependable Registered Nurse to oversee field staff, complete in-home nurse visits, supervise LPNs, and ensure compliance with Missouri DHSS and MO HealthNet requirements.This role is perfect for an RN who enjoys both leadership and hands-on client care, with flexibility to work PRN while helping us grow our client base.\n\nResponsibilities\n

• Perform initial nursing assessments and complete Plans of Care for new clients.\n
• Conduct Authorized Nurse Visits and weekly / periodic supervisory visits for In-Home Services and CDS clients.\n
• Oversee and support LPNs and caregivers to ensure safe, quality care.\n
• Provide clinical oversight for PDN cases, including start-of-care visits, skills validation, and documentation review.\n
• Collaborate with hospitals, rehab centers, and case managers to coordinate client admissions.\n
• Conduct chart audits, ensure compliance with state regulations, and maintain accurate, timely documentation.\n
• Assist with agency quality assurance, incident investigations, and caregiver education.\n\nQualifications\n
• Active Missouri RN license in good standing.\n
• Minimum 1 year of nursing experience in home care, PDN, rehab, hospice, or acute care (pediatric or adult).\n
• Skilled in supervising LPNs and coaching direct-care staff.\n
• Strong assessment, documentation, and organizational skills.\n
• Reliable transportation and willingness to travel throughout the St. Louis metro area.\n
• CPR / BLS certification required.\n
