A company is looking for an RN Care Navigator responsible for case management in kidney health management.
Key Responsibilities
Manage in and outbound calls while delivering exceptional service to members
Educate members on kidney health, dialysis, and transplant options
Coordinate care among patients, healthcare providers, and community resources
Required Qualifications
Active, unrestricted RN license
Bachelor's degree
CCM certification preferred
Three years of experience in case management preferred
Experience in a dialysis or transplant center preferred
