Responsibilities Work in a hospice and palliative care setting serving home health and hospice patients Variable shifts including 8-hour days, evenings, and occasional nights as needed May require on-call coverage for night shift phone calls or home visits Candidates should be prepared to float to different units as needed Provide direct skilled nursing care and educate patients and caregivers under the home health or hospice treatment plan Conduct patient and home evaluations to determine eligibility and safety for home health / hospice services Develop and manage individualized plans of care in collaboration with an interdisciplinary team Coordinate and evaluate care, including discharge and bereavement services for patient families Return travelers must have a 1-year separation from prior PeaceHealth employment Must reside at least 60 miles from the facility to be eligible Some holiday coverage may be required depending on unit needs Modules are not required prior to start Requirements Active RN license for WA or compact state, or proof of WA license application Graduate from an accredited School of Nursing Minimum 1 year of home health or hospice / end-of-life nursing experience required BLS certification required Valid driver’s license and proof of auto insurance required upon hire Experience with Epic EMR within the last year preferred Basic computer proficiency including MS Office applications Must be fully vaccinated (COVID-19), booster not required Benefits High Pay and Bonuses Medical, Dental and Vision Insurance with Day 1 Options 401(k) Plan Weekly Payroll Deposit Free Online CEUs Generous Housing Allowance Travel and Licensure Reimbursements Non-taxed Per Diem and Subsidy Traveler Rewards and Discounts American Traveler Job ID P-622053.
