Job Summary :
Under the general direction of the Manager, Assistant Manager, Clinical Program Manager or other manager / supervisor in KP behavioral health department (e.g. medical social work, psychiatry, addiction medicine), the Behavioral Health Care Coordinator will provide screenings, assessments, coordination and implementation / evaluation of social services to patients and their families in order to aid them in coping with medical, social, emotional and economic difficulties. The coordinator will work with the care team and support multiple programs and departments across the service area. The coordinator will be responsible for making connections to resources and other care managers and staff by sharing important KP and community resources and practices with our members and providers to ensure that the team meets patient care and regulatory requirements.
Essential Responsibilities :
- Upholds Kaiser Permanentes Policies and Procedures, Principles of Responsibilities and applicable state, federal and local laws.
- Under the supervision or direction of a licensed health care provider; interviews, evaluates and assesses patients / care givers / family members social and non-medical needs and creates a care plan based on the patients social, environmental and medical needs, as well as a consideration of the patients strengths.
- Seeks to improve patient / family engagement in management of health care needs, including mental health / psychosocial needs, through early intervention services, motivational interviewing and other interventions within their scope as directed by their manager, supervisor, licensed clinicians / team leader or physicians.
- Provides services in accordance with the plan of care and in accordance with the patients age and developmental needs.
- Selects and utilizes community resources as part of the care plan; makes referrals to and coordinates with professional disciplines, services and community resources and agencies. Ensures that patients secure needed resources.
- May provide or assist in the provision of social therapies using a variety of treatment / intervention methods including coaching, classes, family, individual or group interventions. Refers complicated, highly involved patients and psychosocial problems to licensed clinicians or higher level of care.
- Utilizes supervision and consultation appropriately, as indicated by regular attendance at supervisory / consultation sessions, identifying and discussing problem cases / situations with supervisor and seeking consultation with peers.
- Participates in program / service quality improvement.
- Contributes and shares community resources with colleagues.
- Functions as part of a multi-disciplinary treatment team.
- Case finds and responds referrals / service requests according to established policies and procedures.
- Attends in-service education and professional development programs.
- Performs relevant patient and family advocacy services with KP and the community.
- Provides education to members.
- Establishes and maintains courteous, cooperative relations with the public, patients, and other personnel.
- Promotes, ensures, and improves customer service to internal / external customers by demonstrating skills which are consistent with the organizations philosophy of providing extraordinary customer relations and quality service.
- Documents service activities and patient progress according to KP, regulatory and compliance requirements and maintains statistical data, as well as confidentiality of all patient information.
- Provides case management, triage and linkage to internal / Kaiser and appropriate community services.
- Performs other related duties as required.
Basic Qualifications :
Experience
One (1) year experience in community-based case management or care coordination experience in a mental health, primary care, or acute medical care setting.Education
Bachelors Degree in Social Work, Psychology, Sociology, Counseling, or related field.License, Certification, Registration
N / AAdditional Requirements :
For bilingual required roles, the successful candidate must pass at QBS level II.Bilingual (English / Spanish) Level II required.Preferred Qualifications :
Two (2) years' experience in community-based case management or care coordination experience in a mental health, primary care, or acute medical setting.Completion of Kaiser Permanente customer service assessment and KP basic computer literacy assessment or demonstrated computer aptitude.Demonstrated proficiency in medical terminology, possess medical terminology certification, or medical terminology course completion, or KP medical terminology assessment.Behavioral Health Case Management experience preferred.Notes :
