job_description.job_card.job_descriptionWork in a home health setting serving Chelsea, East Boston, Revere, and Winthrop areas Daily work schedule is 08 : 00-16 : 30 Weekend rotations required once every 6 weeks Patient population ranges from pediatric to geriatric across a variety of diagnoses Must be able to travel between patient homes as assigned EPIC charting system is utilized Responsibilities include providing physical therapy services in patients’ homes for a range of conditions and collaborating with families and interdisciplinary teams Timely and thorough electronic documentation in accordance with agency standards is expected RTO must be disclosed at the time of consideration Local candidates must reside at least 60 miles from central Boston; local pay is reduced Scrubs or professional attire may be required according to agency standards Parking is not validated at Boston locations and public transit or public parking must be used Completion of all onboarding requirements, including references and immunizations, is necessary Active Physical Therapist license in Massachusetts required; no conditional or pending licenses accepted Minimum 1 year of recent home care physical therapy experience required AHA BLS certification required Proof of MA licensure at time of consideration is mandatory Flu and COVID-19 vaccinations are required with no exemptions allowed Ability to travel within designated region to patient residences High Pay and Bonuses Medical, Dental and Vision Insurance with Day 1 Options 401(k) Plan Weekly Payroll Deposit Free Online CEUs Generous Housing Allowance Travel and Licensure Reimbursements Non-taxed Per Diem and Subsidy Traveler Rewards and Discounts
