Seeking a dedicated Registered Nurse to serve as MDS Coordinator at a skilled nursing facility. The ideal candidate will have experience in long-term care, with knowledge of MDS assessments, Medicare, and care planning processes. This role requires strong attention to detail, clinical expertise, and the ability to collaborate with interdisciplinary teams.

Patient Population : Skilled Nursing / Long-Term Care residents

FTE : 1.0 (40 hours / week)

Shift : 8 : 00 AM – 4 : 30 PM, Monday–Friday

Call : Clinical on-call every 3rd week; weekend rotation approximately every 12 weeks

Pay Range : Starting at $34.50 / hr

Sign-On Bonus : $15,000

Relocation Bonus : None

Licenses Required : RN

Experience Preferred : Minimum 1 year RN in LTC with MDS experience
