Job Title : Hospice Team Coordinator (Scheduler)
Position Summary :
The Hospice Team Coordinator plays a key role in supporting the interdisciplinary team by managing staff scheduling, coordinating patient care assignments, and ensuring seamless communication across clinical and administrative teams. This position ensures that patients and families receive timely and consistent hospice services through efficient scheduling of nurses, aides, social workers, chaplains, and other care team members.
Responsibilities :
Develop, maintain, and adjust staff schedules to ensure appropriate coverage for hospice patients in all service areas.
Assign and update daily visits for RNs, Social Workers, CNAs, Chaplains, and other team members, balancing workload fairly and efficiently.
Monitor call-offs, vacation requests, and staffing changes, adjusting schedules as needed.
Communicate schedule updates promptly to staff and leadership.
Collaborate with the interdisciplinary team to ensure patient needs are met in a timely manner.
Maintain accurate records of schedules, assignments, and visit coverage.
Participate in daily / weekly staffing meetings and provide updates on coverage gaps or challenges.
Assist with onboarding and orientation of new clinical staff to scheduling systems and processes.
Provide excellent customer service to employees, patients, and families by addressing scheduling questions and concerns professionally.
Support compliance with organizational policies, state / federal regulations, and hospice standards of care.
Qualifications :
High school diploma or equivalent required; associate's degree preferred.
Previous experience in healthcare scheduling, staffing coordination, or hospice / palliative care setting strongly preferred.
Strong organizational and time-management skills with ability to prioritize and multitask.
Excellent communication and interpersonal skills.
Proficiency in scheduling software, EMR systems, Microsoft Office Suite (Word, Excel, Outlook).
Ability to work independently while being a strong team player.
Compassion, professionalism, and sensitivity when working with patients, families, and staff.
Working Conditions :
Full-time, office-based role
