FEES List typical procedures performed on unit(s) : Dysphagia Eval, SLC eval Covers IRF evals and follow up sessions Best personality Fit : Flexibility, Self-awareness REQUIRED CERTIFICATIONS : Active SLP license, BLS Floating Area(s) : IRF and Acute Care Dress Code : Scrub Color / Attire - Navy Blue Unit Guidelines / Policies : Please feel free to attach unit policies and guidelines. Need to see stroke evals within 24 hours from time of consult, Productivity requirement is 68.75 for acute and 75% for rehab, documentation and charges should be completed before the shift ends.
