State Licensed Director of Clinical Excellence

A company is looking for a Director, Clinical Excellence (Post Acute, VBC).

Key Responsibilities

Lead the operational delivery of the post-acute care program to ensure coordinated patient transitions

Build and maintain partnerships with skilled nursing facilities (SNFs) and manage clinical performance expectations

Track and report key performance indicators (KPIs) and collaborate with senior leadership to improve quality of care

Required Qualifications

Licensed RN, NP, or PA

Bachelor's degree in Nursing or a related field; Master's degree preferred

Minimum 3 years of clinical leadership in care management or post-acute settings

Experience in value-based care and data-driven quality improvement

Proven ability to lead interdisciplinary teams in a fast-paced environment
