Job Summary :
We are seeking a compassionate and skilled Primary Care Nurse Practitioner (NP) to join Hamilton Medical Group under the supervision of Dr. Ashley West, a board-certified Family Medicine Physician. The NP will provide high-quality, patient-centered primary care services to adults, collaborating closely with a collaborating physician to ensure comprehensive and coordinated care. This role involves diagnosing and treating a wide range of acute and chronic conditions, promoting preventive care, and fostering strong patient relationships in a supportive, team-oriented environment.
Key Responsibilities :
- Conduct comprehensive patient assessments, including medical history, physical exams, and diagnostic evaluations.
- Diagnose and manage acute and chronic illnesses, such as infections, diabetes, hypertension, and respiratory conditions.
- Order, interpret, and follow up on diagnostic tests (e.g., lab work, imaging).
- Prescribe medications and therapies in accordance with state regulations and practice protocols.
- Provide preventive care, including health screenings, immunizations, and patient education on wellness and disease prevention.
- Collaborate with Dr. Ashley West to develop and implement individualized treatment plans.
- Document patient encounters accurately and timely in the electronic health record (EHR) system.
- Coordinate referrals to specialists and other healthcare providers as needed.
- Participate in patient follow-ups and care coordination to ensure continuity of care.
- Educate patients and families on health conditions, treatment options, and lifestyle modifications.
- Adhere to evidence-based guidelines, clinic policies, and state regulations governing nurse practitioner practice.
- Contribute to quality improvement initiatives and team meetings to enhance patient care delivery.
Qualifications :
Master’s or Doctoral degree in Nursing from an accredited Nurse Practitioner program.Current, unrestricted Nurse Practitioner license in Louisiana.National certification as a Family Nurse Practitioner (FNP) or Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP).Minimum of 1-3 years experience in primary care or family medicine preferred; new graduates with strong clinical training may be considered.Current BLS certificationProficiency in EHR systems and basic computer skills.Strong clinical assessment and diagnostic skills.Excellent communication, interpersonal, and teamwork skills.Commitment to patient-centered care and cultural sensitivity.Work Environment :
