Weare currently seeking a dedicated and compassionate Primary Care Nurse Practitioner (NP) to join our team and work closely under the supervision of our Family Medicine Physician.

Job Summary :

We are seeking a compassionate and skilled Primary Care Nurse Practitioner (NP) to join Hamilton Medical Group under the supervision of Dr. Ashley West, a board-certified Family Medicine Physician. The NP will provide high-quality, patient-centered primary care services to adults, collaborating closely with a collaborating physician to ensure comprehensive and coordinated care. This role involves diagnosing and treating a wide range of acute and chronic conditions, promoting preventive care, and fostering strong patient relationships in a supportive, team-oriented environment.

Key Responsibilities :

• Conduct comprehensive patient assessments, including medical history, physical exams, and diagnostic evaluations.
• Diagnose and manage acute and chronic illnesses, such as infections, diabetes, hypertension, and respiratory conditions.
• Order, interpret, and follow up on diagnostic tests (e.g., lab work, imaging).
• Prescribe medications and therapies in accordance with state regulations and practice protocols.
• Provide preventive care, including health screenings, immunizations, and patient education on wellness and disease prevention.
• Collaborate with Dr. Ashley West to develop and implement individualized treatment plans.
• Document patient encounters accurately and timely in the electronic health record (EHR) system.
• Coordinate referrals to specialists and other healthcare providers as needed.
• Participate in patient follow-ups and care coordination to ensure continuity of care.
• Educate patients and families on health conditions, treatment options, and lifestyle modifications.
• Adhere to evidence-based guidelines, clinic policies, and state regulations governing nurse practitioner practice.
• Contribute to quality improvement initiatives and team meetings to enhance patient care delivery.
