Job Summary
In this role, you will primarily be creating and managing a plan of care for individual patients. You are a Licensed Practical Nurse (LPN) who operates with great attention to detail and a big heart. You take pride in creating personalized care plans for patients and are able to observe and communicate effectively in your patient's best interest. To succeed, you should have demonstrated knowledge and skill in current nursing practices as well as a strong penchant for solving problems.
We are seeking a Licensed Practical Nurse who lives in the Edina community to join our team. We're looking to hire a full time or part time nurse who is competent to perform the following task including but not limited to :
Responsibilities
Performs focused nursing assessment of Residents to determine appropriate interventions to maintain or improve the Residents health status.Provides interventions for Residents with episodes of potentially life-threatening instability.Provides nursing visits and direct care to Residents in the resident's home or assisted living as directed by client Care Plans.Sets up medications for Residents according to the medication management program, as indicated.Coordinates medical appointments for Residents, including follow-up with any new orders or changes after hospital visit with direction from Nursing case manager as neededAccepts and implements prescriber orders with direction as neededResponds to phone calls from other providers related to Residents with RN direction as neededDocuments, as indicated, in the clinical recordCommunicates with families, resident representatives, case managers and other health care providers regarding the Residents as neededIs aware of and uses good safety practices, including but not limited to :Body mechanics
Falls preventionReporting of defective equipment or hazardsWiping spills or wet floors immediatelyUsing Standard Precautions and appropriate infection control measuresKnowledge of emergency proceduresReporting incidents / accidents to the Clinical Nurse Supervisor or Director on the day of occurrenceParticipates in care conferences with other members of the health care teamMaintains absolute confidentiality of all information pertaining to Residents, families, and employeesPromotes a safe and comfortable environment for the ResidentsDemonstrates excellent written and verbal communication skills12 Hour Shifts
Week 1 Wednesday - Friday
Morning 7 AM - 7 PMWeek 2 Monday - Friday, Sunday
