Pay Range
Base pay : $22.28 – $33.42 per hour. Pay is based on your skills and experience – discuss with your recruiter for details.

Responsibilities
Serve as the initial point of contact for individuals seeking services; conduct structured intake interviews to gather preliminary information and assess general needs.Verify client eligibility through documentation review, data collection, and agency-defined criteria.
Accurately document and maintain intake information in client databases (e.g., Extended Reach or equivalent platforms).Coordinate and schedule initial appointments or warm hand-offs with assigned case managers, navigators, or direct service staff.Offer brief guidance to help clients understand available services and navigate intake requirements, without assuming responsibility for service plan implementation.Maintain knowledge of internal programs and referral procedures to support appropriate service routing.Collaborate with internal departments and community partners to ensure continuity and accessibility of care.Support front-end administrative functions such as client follow-up calls, reminder scheduling, or record audits for completeness.Uphold documentation confidentiality and data accuracy standards in alignment with HIPAA, agency policy, and program-specific guidelines.Qualifications
1 year of related experience in client intake, eligibility screening, program coordination, or administrative support in a human services setting.Lived experience that enhances relatability to clients being servedBachelor's degree in social work, human services, psychology, or related field; substantial relevant experience will be considered in place of degree.
Fluency in English, both spoken and written.Basic / Intermediate / Advanced mastery of business applications and productivity suites.Demonstrates resilience, awareness and self-regulation in navigating challenges, conflict and obstacles / barriers.Thrives in a diverse and multicultural environment; works with cross-functional teams to develop and implement strategies to enhance end-user experiences throughout the service lifecycle.Demonstrates exceptional commitment to serving and advocating for diverse and marginalized populations.Successfully completes tasks in a moderately noisy environment.Interact with others and perform tasks in-person and using technology, with long periods of stationary activity and moderate to increased physical activity including and not limited to sitting, standing and walking.Moves equipment / materials weighing up to 25 pounds.Travels between worksites occasionally.Works in a variety of settings, including and not limited to LCSNW office locations, partner agency and community-based settings, and faith-based settings such as churches.Meets LCSNW criminal background clearance requirements and other job-relevant clearances as required by the State or program.Thank You
