Job Title : Field RN - Home Health Admissions

Company : Home Health Care, Inc.

Compensation : $150 - $200 per visit + mileage reimbursement!

If you love hitting the road, meeting patients in their homes, and creating care plans from day one, this is your role. As a Field RN for Admissions , you'll thrive in a flexible, fast-moving environment where you set the tone for patient care.

Pay : Earn $150-$200 per visit, plus mileage
Mission : Guide patients through the start of their home health care journey

Job Responsibilities :

Complete Start of Care OASIS admissions

Create and implement care plans

Document accurately and efficiently

Collaborate with care team for seamless transitions

Requirements :

Active Minnesota RN license

Prior OASIS and SOC experience

Ability to work independently and communicate clearly

MOC Equivalents Include :

Army : 66B

Navy : 1960

Air Force : 46SX

Marines : MED

Coast Guard : HS
