Qualifications :
Education : Bachelor of Science in Nursing (BSN) preferred.Licensure : Current and unencumbered Registered Nurse (RN) license in the state of practice.Experience : A minimum of two years' experience in a management or supervisory role within a home care or home health setting is highly preferred.Technical Skills : Strong knowledge of professional standards, Medicare Conditions of Participation, and federal, state, and local regulations.Soft Skills : Except for leadership, mentorship, communication, and interpersonal skills. Ability to serve as a liaison between patients, families, and staff.Key Responsibilities :
Manage and oversee patient care services, ensuring compliance with all regulatory standards.Lead, mentor, and support nursing staff, fostering professional growth and enhanced patient outcomes.Collaborate with interdisciplinary teams to develop and implement individualized patient care plans.Ensure accurate and timely documentation of patient care activities and outcomes.Conduct regular audits of patient records and care plans to uphold quality assurance.Act as a liaison among patients, families, and healthcare providers to ensure seamless care coordination.Engage in quality improvement initiatives and participate in ongoing professional development.If you are ready to take the next step in your nursing career and make a difference in the healthcare industry, we want to hear from you!
