Location : Clark or Washoe County, NV (Field visits required)

Schedule : Monday-Friday, 8 : 00 AM - 5 : 00 PM

Type : Contract | W2 | Weekly Pay | Benefits Available

Travel Reimbursement : Gas mileage reimbursement provided

Position Overview :

We are seeking a compassionate and experienced Behavioral Health Care Manager (BHCM) to provide care management services to members with complex behavioral health needs. This hybrid role combines remote work with field visits to members' homes, provider offices, and community settings within Clark or Washoe County, NV.

Key Responsibilities :

Coordinate behavioral health care for members across settings and services.

Collaborate with members, families, providers, facilities, and internal care teams to develop and implement individualized care plans.

Provide health education, coaching, and treatment decision support.

Conduct field visits to members' homes, provider offices, or inpatient facilities as needed.

Ensure compliance with NCQA and state regulations for case management services.

Maintain accurate documentation and case records.

Promote quality, self-management, and cost-effective care strategies.

Perform additional duties as assigned.

Required Qualifications :

RN (Registered Nurse)

LMFT (Licensed Marriage and Family Therapist)

CPC (Certified Professional Counselor)

LCSW (Licensed Clinical Social Worker)

Minimum 3 years of experience in a behavioral health setting or direct patient care

At least 1 year of case management experience (preferred)

Knowledge of managed care environments and patient care delivery

Familiarity with state programs and community-based services

Strong problem-solving skills and initiative

Preferred Certification :

CCM (Certified Case Manager) - preferred but not required
