Coordinate patient care delivery with the interdisciplinary team, ensuring timely and accurate documentation. Foster a respectful, inclusive, and collaborative clinical environment. Participate in performance improvement initiatives to promote safe, quality care. Complete all mandatory professional education and competency assessments. Serve as a mentor or preceptor for other staff members and nurses, promoting a qualified workforce and healthy work environment. Participate in evidence-based practices and research to continuously improve patient care. Uphold policies and clinical competencies in line with healthcare regulations and safety standards. Contribute to the data collection process, support clinical inquiries, and integrate new evidence into practice. Plan and manage patient care effectively across the care continuum. Collaborate in the development of care protocols to improve outcomes such as reduced readmission rates. Ensure efficient and effective use of resources, including materials and staff. Job Description :
Proactively plans for the care of patients across the care continuum in the course of giving direct patient care.
Participates in the development of protocols and procedures when called upon or through self-initiation in collaboration with care managers and other members of the health care team to achieve best practice outcomes (i.e., decrease in re-admission rates; avoidable days; adverse events; etc).
