Position Summary
The Licensed Practical Nurse (LPN) delivers safe, effective, and compassionate nursing care to patients on the 28-bed Medical-Surgical Unit under the supervision of a Registered Nurse (RN) and / or physician. The LPN collaborates with the healthcare team to support patient recovery, provide education, and ensure continuity of care.
Key Responsibilities
Patient Care
Assist with patient admission, transfer, and discharge processes.
Collect and document patient health information, including vital signs, intake and output, and daily assessments.
Administer prescribed medications and treatments within scope of practice and according to hospital policy.
Provide wound care, specimen collection, and other delegated clinical tasks.
Support patient mobility, personal hygiene, nutrition, and comfort needs.
Monitor patients for changes in condition and promptly report to the RN or provider.
Clinical Skills & Safety
Perform technical nursing procedures (e.g., IV therapy if certified, catheter insertion, tracheostomy care).
Maintain accurate, timely documentation in the electronic health record (EHR).
Follow infection prevention, fall prevention, and patient safety protocols.
Assist with emergency response procedures and interventions as directed by the RN.
Collaboration & Communication
Work closely with RNs, physicians, therapists, and other interdisciplinary team members.
Provide clear, respectful, and professional communication with patients, families, and staff.
Participate in shift report and unit huddles to ensure safe handoff of care.
Advocate for patients’ dignity, respect, and inclusion in their care.
Professionalism & Growth
Maintain current licensure and required certifications-BLS
Engage in continuing education and professional development opportunities.
Participate in quality improvement initiatives and safety programs.
Serve as a mentor or resource for nursing assistants and new LPN staff.
Qualifications
Education : Graduate of an accredited Practical Nursing program.
Licensure : Current LPN license in [State].
Certifications : BLS required
Experience :
Prior acute care or medical-surgical experience preferred.
New graduates considered with appropriate orientation.
Strong clinical skills, attention to detail, and ability to work in a fast-paced environment.
Physical & Work Environment Requirements
Ability to lift, move, or transfer patients and equipment (up to 50 lbs).
Frequent standing, walking, and bending.
Exposure to infectious diseases, hazardous materials, and stressful situations.
Shift work including days, nights, weekends, and holidays as required.
