Nationwide Therapy Group is seeking a travel nurse RN Hospice for a travel nursing job in Fort Worth, Texas.

Position : RN Case Manager – Hospice

Location : Fort Worth Office

Shift : 8-Hour Days, Monday–Friday

Start Date : ASAP

Contract Length : 13 Weeks

Description

The RN Case Manager provides hospice care in accordance with National Hospice and Palliative Care Organization standards, focusing on holistic, patient- and family-centered care.

Responsibilities

  • Assess and identify patients’ and families’ physical, psychosocial, environmental, safety, and developmental needs.
  • Develop and individualize patient care plans based on assessments.
  • Document problems, nursing assessments, goals, interventions, and outcomes in the EMR.
  • Assess pain and develop individualized pain management plans.
  • Anticipate, prevent, and treat undesirable or secondary symptoms.
  • Participate in interdisciplinary team (IDT) meetings to review and update plans of care.
  • Educate patients, caregivers, and volunteers on interventions, medications, and side effects.
  • Provide compassionate end-of-life care, supporting comfortable and dignified dying.
  • Support patients’ and families’ spiritual and cultural needs.
  • Coordinate patient and family services, ensuring continuity of care.
  • Maintain accurate, complete, and compliant electronic medical records.
  • Collaborate with the care team to ensure quality hospice and palliative care delivery.

    • Requirements

  • Experience : Hospice experience required
  • License : Active RN license
  • Skills : Strong assessment, documentation, and communication skills; ability to provide holistic and compassionate care
