POSITION SUMMARY

LPN will collaborate with quality team and primary care providers to improve : risk diagnosis coding, medication reconciliations, statin measures, disease specific patient education, and transitions of care.

POSITION QUALIFICATIONS

Minimum Education

Graduation from an accredited LPN program

Minimum Experience

1 year hospital experience

Required Registration / License / Certification

BLS obtained within 90 days of hire or transfer

Licensed Practical Nursing license

Preferred Education

Preferred Experience

• 1+ years clinical experience
• Proficiency with Microsoft Excel
