Homecare Nurse Liaison - LPN at The HomeAides is a Non-Medical position.
Maximize referral growth and client satisfaction as a Homecare Nurse Liaison at The HomeAides. In this pivotal role, you'll strategically build and nurture relationships with key referral partners while leading tailored memory care programs that drive client retention and positive outcomes. Alzheimer’s / Dementia Care Leadership :
Oversee and enhance care programs specifically tailored to individuals with Alzheimer’s or related dementias.
Provide ongoing training and support to caregivers on dementia best practices, communication techniques, and behavior management.
Serve as an internal subject matter expert on memory care, ensuring quality and consistency in service delivery.
Conduct client and family assessments to create customized care plans that promote safety, dignity, and independence.
Monitor client progress and adjust services as needed in collaboration with families and care staff.
Business Development and Relationship Management :
Identify, develop, and maintain strong B2B relationships with referral sources including hospitals, assisted living facilities, senior centers, care managers, and medical practices.
Represent the company at community events, professional networking groups, and educational seminars related to senior care and dementia support.
Collaborate with the marketing team to create targeted outreach campaigns and educational content that highlights the company’s dementia care expertise.
Generate qualified leads and assist in converting them into clients through consultative engagement and needs-based recommendations.
Research and initiate B2B prospecting activities for new referral sources via calls, email, virtual or in-person meetings, social media, etc. to target opportunities to build new relationships with new decision-makers.
Field, qualify and convert inbound referrals and client intakes coming from various referral sources, marketing, advertising and promotional initiatives.
Consistently generate qualified opportunities to achieve department targets and drive revenue growth from inbound and outbound B2C / B2B prospecting opportunities.
Meet sales expectations of 15 onboarded clients per month
Align with our leadership and marketing teams to strategize methods and tactics to target companies within specific industries and demographics.
Become comfortable and consistent in verbally delivering our various service offerings and value propositions to a wide range of inquiring consumers or referral sources in a way that builds trust / rapport in the end-user feeling comfortable with utilizing our services.
Conduct various lead nurturing, lead follow-up and lead management activities that ensure ongoing communication throughout the sales process and customer journey from initial inquiry to conversion.
Stay up-to-date on healthcare, home care and other relevant industry related trends, news, etc. that allow us to translate those learnings into actionable plans.
Create and maintain task-specific supported processes and organizational skills to ensure efficiency during day-to-day responsibilities when possible.
Lead and account maintenance in our CRM - managing updated notes, account updates, contact information to ensure accurate data entry on all accounts.
Cultivate and maintain relationships with key decision makers at hospitals, skilled nursing / rehab facilities, assisted living facilities, memory care centers to ensure consistent collaboration with the coordination of care for clients in need.
Must have an understanding of the duties and responsibilities of caregivers in a non-medical home care setting. In addition, having an understanding of medical diagnosis and disease process to include conditions related to Alzheimer / Dementia, CHF, COPD, Diabetes ,Parkinson's and end of life care.

JOB REQUIREMENTS
Bachelor’s degree in social work, healthcare administration, nursing, gerontology, or a related field (required).
Certification or advanced training in dementia care (e.g., CDP – Certified Dementia Practitioner) preferred.
Minimum of 3 years of experience in senior care, with at least 2 years in Alzheimer’s / dementia-specific roles.
Proven experience in business development, sales, or relationship management within the healthcare or senior services sector.
Excellent communication, training, and leadership skills.
Strong organizational and time management abilities.
Valid driver’s license and reliable transportation for community travel.
