    Summary

    This role involves providing comprehensive primary and specialty medical care, working collaboratively with our team in a clinical setting in Uniontown.

    Responsibilities

  • Patient Care and Assessment : Conduct patient histories, perform physical examinations, and assess presenting illnesses, risk factors, and psychosocial situations.
  • Diagnosis and Treatment
  • Order and interpret diagnostic tests, develop and implement treatment plans, and diagnose both acute and chronic conditions.

  • Medication and Procedures
  • Prescribe and administer medications and injections in accordance with state laws and practice guidelines. Perform minor in-office procedures and manage wound care.

  • Patient Education : Instruct and counsel patients and their families on disease management, preventive care, and healthy lifestyle choices.
  • Collaboration
  • Work closely with physicians and other team members to ensure continuity of care and make appropriate referrals to specialists.

  • Documentation
  • Maintain accurate and timely electronic medical records for all patient encounters.

    Qualifications

  • Education
  • Advanced degree from an accredited Nurse Practitioner or Physician Assistant program.

  • Licensure and Certification
  • Current, valid Pennsylvania state licensure as a Certified Registered Nurse Practitioner (CRNP) or Physician Assistant-Certified (PA-C).

  • Prescriptive authority from the state of Pennsylvania.
  • Current Drug Enforcement Agency (DEA) licensure.
  • Basic Life Support (BLS) certification.
  • Skills

  • Strong clinical judgment and critical thinking skills.
  • Excellent interpersonal and communication skills to effectively interact with patients and colleagues.
  • Ability to work both independently and as part of a collaborative healthcare team.
  • Proficiency in using electronic medical records (EMR) systems.
