Healthcare Sales & Marketing Patient Care Liaison / Navigator
About Our Company
Digital Health Information is a new and innovative system that provides dedicated care to the Long-Term Care community, specifically to patients residing in their own home. We are an independently owned LTC pharmacy, telehealth physician practice (not open to the general public), and remote patient care facility. As a new ecosystem we are working to expand our operations nationwide, one state at a time, and currently operate in Pennsylvania and New Jersey localities.
OVERVIEW
Our Patient Care Navigator plays a crucial role in connecting patients within the community with our services and other resources. This position requires an individual with a passion for public health, community engagement, sales and marketing skills, as well as experience and knowledge of the healthcare landscape. The Patient Care Navigator outreaches to prospective patients via phone calls and community events. They serve as correspondents in the field going out to patient homes for prospective patient intakes, home assessments, and device deployment / installation.

Proposed Schedule : Mon- Fri 9 : 00a-5 : 00pm (40 hours per week)

$18.00 - $24.00 per hour (negotiable based on experience) + quarterly marketing bonuses
