Your Role as a Home Health Nurse :

In this rewarding position, you will :

  • Provide compassionate home-based care to patients recovering from illnesses, injuries, or chronic diseases.
  • Collaborate with a team of physicians and oversee LPNs, CNAs, and aides in delivering exceptional patient care.
  • Conduct patient assessments, document progress, and communicate changes to physicians.
  • Educate patients and their families on care plans, medications, and home safety.
  • Ensure patient goals are met and coordinate discharge from services.

    • What We’re Looking For :

    To join our team, you’ll need :

  • An active, unrestricted Registered Nurse (RN) license in the State of Michigan (BSN not required).
  • At least one year of clinical experience as a Registered Nurse.
  • Current CPR Certification.
  • Reliable transportation, a valid state driver's license, and current car insurance.
