Patient Care Coordinator

A company is looking for a Patient Care Coordinator.

Key Responsibilities

Manage requests for services from members and providers, coordinating care needs

Follow up on referrals and prior authorizations, providing administrative support to clinical staff

Assist with discharge planning and maintain confidentiality of member information

Required Qualifications

High School Diploma / GED or equivalent work experience

Must be 18 years of age or older

2+ years of work experience in a medical office or medical field

Experience with computers and Windows-based programs

Knowledge of medical terminology
