🌿 Hospice Registered Nurse (RN) Case Manager

Muskegon to Grand Rapids

Full-Time | Monday–Friday, 8a–5p | $5,000 Sign-On Bonus!

🌼 What You’ll Do

Perform thorough patient assessments and develop individualized care plans

Coordinate with an interdisciplinary team to deliver high-quality hospice care

Educate and support patients, families, and caregivers on hospice philosophy, pain control, and symptom management

Monitor changes in patient condition and communicate updates promptly

Provide emotional and spiritual support during end-of-life care

Serve patients in home settings, nursing facilities, and assisted living communities

🩺 What You’ll Need

Graduate of an accredited nursing program (Diploma, ADN, or BSN)

Current RN license in the state of practice

1+ year of clinical experience (hospice or home health preferred)

Reliable transportation , valid driver’s license, and proof of auto insurance

Ability to lift, move, and assist patients (up to 50–100 lbs)
