Position Summary
The Intake Registration Clerk interviews incoming patients or their representatives, prepares registration forms for signature, verifies insurance eligibility, benefits, and authorizations, and processes charges into the computer system for services rendered. This role also includes functions of cashier, telephone operator, and health information clerk, and is responsible for completing authorizations and referrals for medical services.
Essential Functions
Registration & Intake
- Obtain and verify accurate registration data :
Social, financial, and personal information
Insurance forms, ID cards, and patient identificationManage the registration process :Determine services needed (e.g., pre-registration)
Obtain necessary signaturesExplain clinic policies and third-party payer proceduresComplete and explain MSP forms for Medicare patientsSecure proper authorizations prior to servicesDocument account activity and financial statusHealth Information Management (Paper Charts)
Assemble charts in proper orderCreate filing charts with correct terminal digit stickersGenerate medical record deficiency slipsMaintain filing systems in designated areasConfidentiality & Compliance
Ensure confidentiality and security of patient health informationProvide feedback on admission and billing process qualityObtain and verify authorizations for release of recordsAssemble nightly documentation for HIM codingRespond promptly to departmental inquiriesClerical & Communication Duties
Answer phone calls efficiently and courteouslyType and compile necessary reportsFiling & Scanning
Scan lab and ancillary reports into medical recordsFile completed charts in permanent system (terminal digit order)Organize incomplete charts in designated areaMaintain filing system accuracy and completenessFinancial Responsibilities
Balance cash drawersAccept payments (cash, credit, checks)Generate receipts for patients and guarantorsAuthorizations & Referrals
Complete authorizations and referrals for appointments and proceduresReview medical history and insurance coverage for approvalContact referring physicians for additional informationInput and update patient information in EMRMonitor schedules and maintain referral documentationReview referral / authorization details with patientsRemind patients of scheduled appointmentsAuthorization Tasks :
Process prescription and procedure authorizationsCheck patient eligibility and benefitsSchedule proceduresComplete referralsEnter all data into EMRPolicy & System Usage
Demonstrate understanding of Admission Policies and ProceduresRoutinely use Admissions Policy manual and reference materialsUtilize HIS Affinity system to full capacity and adapt to system enhancementsOther Responsibilities

High school diploma or equivalentMinimum 3 years experience in patient registration (hospital preferred)Knowledge of medical terminology preferredMathematical ability to review financial recordsProficient with 10-key adding machine, typing, and computer systemsAccuracy prioritized over speedFamiliarity with filing systems and copy machinesBilingual (Spanish / English) preferredAGE OF POPULATION SERVED :
