We're seeking an experienced Hospice Quality Manager who brings a deep understanding of hospice care, a commitment to excellence, and a passion for ensuring every patient receives compassionate, compliant, high-quality service. This is a hands-on leadership role for an experienced Hospice RN who thrives on driving performance improvement, supporting clinical teams, and ensuring survey readiness while keeping patients' comfort and dignity at the center of everything we do.

What You'll Do

  • Oversee and enhance quality of care across hospice services through regular audits, data analysis, and collaboration with clinical teams.
  • Lead and facilitate QAPI meetings, survey readiness efforts, and follow-up plans of correction when necessary.
  • Conduct focused clinical audits and staff shadowing to identify opportunities for improvement and celebrate best practices.
  • Partner closely with Compliance, Education, and Leadership teams to maintain regulatory compliance and a culture of continuous improvement.
  • Deliver training and guidance to field staff, ensuring adherence to hospice regulations and The Care Team's standards of care.
  • Analyze quality metrics and report outcomes that drive meaningful change in patient and family experiences.

What We're Looking For

  • Registered Nurse (RN) with hospice field experience (required).
  • Strong working knowledge of hospice regulatory and compliance requirements.
  • Proven ability to analyze data, lead quality initiatives, and support performance improvement.
  • Exceptional communication and coaching skills to guide and support interdisciplinary teams.
  • Reliable transportation and valid driver's license.
