Role Description
The Nutritionist / Dietitian provides medical nutrition therapy, develops personalized dietary plans, and educates clients on healthy eating habits for Medicaid populations, including individuals with multiple chronic illnesses, mental health conditions, substance use disorders, and pregnant mothers. The ideal candidate will have at least five years of experience delivering nutrition interventions in community-based, clinical, or social service settings. This role requires a compassionate, patient-centered approach to support vulnerable populations in achieving better health outcomes through tailored nutrition plans and education.
Responsibilities
- Conduct comprehensive nutritional assessments for individuals with complex medical and social needs.
- Develop and implement personalized nutrition care plans addressing chronic disease management, mental health conditions, prenatal nutrition, and substance use recovery.
- Collaborate with care coordinators, social workers, and healthcare teams to ensure integrated support for clients.
- Provide nutrition education and counseling for clients and their families, promoting sustainable dietary habits.
- Monitor and track clinical outcomes related to diet and lifestyle interventions, adjusting plans as needed.
- Support Medicaid clients in accessing nutritional resources, food benefits (SNAP, WIC), and meal programs.
- Maintain accurate and timely documentation of client interactions following the program standards.
- Participate in community outreach, workshops, and group education sessions to promote and support better health habits.
- Stay updated on evidence-based nutritional practices for populations with chronic disease, mental health needs, and substance use disorders.
- Ability to support the enhanced service department with other tasks assigned to advance the program.
Qualifications & Skills
Licensed as a Nutritionist or Registered Dietitian (RD / RDN) in New York State.Minimum of 5 years working with Medicaid populations, including individuals with chronic illnesses, mental health conditions, substance use disorders, and pregnant mothers.Expertise in medical nutrition therapy (MNT), chronic disease management, prenatal nutrition, and behavioral health integration.Strong interpersonal and communication skills for working with diverse, at-risk populations.Ability to develop culturally competent and trauma-informed nutrition interventions.Experience with electronic medical records (EMRs) and Medicaid documentation requirements.Ability to collaborate with interdisciplinary teams in social services and healthcare settings.Bilingual in Spanish, Haitian Creole preferred.
