Join Our Team as a DD Case Manager

At KVC Health Systems, we are passionate about creating meaningful connections and improving the lives of individuals with developmental and intellectual disabilities. We are seeking a DD Case Manager to provide vital support, training, and resources to Shared Living Providers (SLPs) who care for individuals in a home setting.
About the Role
The DD Case Manager plays a key role in supporting SLPs and ensuring quality care for individuals placed in Shared Living homes. This position requires leadership, strong communication skills, and a commitment to excellent service and advocacy.
This is a hybrid position offering flexibility on in-office days, with some required in-person meetings and home visits.
Key Responsibilities
Training (20%) : Deliver initial and ongoing training for SLPs, provide individualized support, and coordinate with community partners to enhance care.
Ongoing Support & Crisis Management (40%) : Offer around-the-clock support to SLPs, conduct home visits, maintain regular contact, attend meetings, and advocate for individuals and providers.
Documentation (40%) : Ensure timely and accurate completion of all required records, including data collection, audits, reports, and participation in Rights Review Committee meetings.
Qualifications
Bachelor’s degree in Social Work, Human Services, or a related field.
At least 2 years of experience in Developmental Disabilities, Child Welfare, Juvenile Justice, or Mental Health Assessment and Treatment.
Intermediate skills with Microsoft Word, Excel, Outlook, and general typing / keyboarding.
At least 21 years of age.
Valid driver’s license and auto insurance.
Preferred Experience
Direct experience providing services to individuals with disabilities.
Familiarity with state-mandated databases (e.g., Therap).
