Community Options, Inc . is a national non-profit agency providing residential group home, day habilitation and supported employment services to individuals with disabilities in 12 states.
We are seeking a Licensed Practical Nurse (LPN) in Council Bluffs, IA. The L.P.N. / L.V.N. delivers care to individuals in a community setting utilizing the nursing process of
assessment, planning, intervention, implementation, and evaluation. This person will interact with individuals, significant others, and health team members while maintaining the standards of professional nursing.
Starting Pay Rate : $25.00 / hourly
Responsibilities
- Provide treatment and health care procedures ordered by a physician and required by the standards of professional practice or law
- Ensure individuals attend medical appointments per their Individual Support Plan (ISP)
- Advise supervisor of all concerns and issues related to the provision of nursing services
- Create and distribute medical / health reports detailing general health, health care goals, and physician recommendations
- Develop and update Individual Support Plans (ISP) / Person Centered Plans (PCP) with an interdisciplinary team to include any concerns, observations, and behavioral developments
- Train employees on the medical needs of assigned individuals
- Monitor health and medical conditions of individuals and refer for medical services as needed
- Monitor the use of medications through medication administration logs
- Ensure health service supplies are available and replenished as needed
- Ensure program documentation and billable records are completed accurately and timely
- Build a support network for individuals by developing and maintaining relationships with community members, businesses, and families
- Participate with individuals in community, social, and recreational activities
- Participate in circles of support for personal futures planning
- Follow strict OSHA procedures regarding blood borne pathogens
- Additional tasks and responsibilities may be assigned
- Employee must cooperate with the license and department staff in any inspection, inquiry, or investigation
- Employment is contingent upon successful completion of checks of criminal background, central registry, child abuse registry, and drug testing
Minimum Requirements
Bachelor’s degree or equivalent experience in nursing fieldMinimum of one year of experience providing nursing services to individuals with disabilitiesValid nursing licenseValid driver’s license with a satisfactory driving recordComplete all state and agency required training per state guidelinesAbility to calculate figures related to medication dosagesUnderstanding and commitment to community-based support for persons with disabilitiesAbility to exercise good judgment and remain calm in crisis situationsAbility to establish a comfortable and supportive relationship with an individual receiving supportsProficient with Microsoft OfficeExcellent verbal and written communication skillsAbility to effectively present facts and recommendationsAbility to effectively communicate with program participants, third-party agencies, staff, and members of the communityExcellent time management skillsWorking Conditions
