Senior Physical Therapist - Long-Term Care Specialist

Pawling, NY | Full-Time | Direct Hire

Position Overview

We're seeking an experienced Physical Therapist to join our premier rehabilitation and skilled nursing facility in Pawling, NY. As a vital member of our therapy team, you'll deliver exceptional care while working in a collaborative, resident-centered environment that emphasizes professional growth and work-life balance.

Essential Responsibilities

  • Design and implement individualized treatment plans for complex rehabilitation cases
  • Perform comprehensive patient evaluations and progress assessments
  • Deliver evidence-based therapeutic interventions focusing on functional mobility and pain management
  • Lead rehabilitation programs for post-acute and long-term care residents
  • Coordinate care plans with interdisciplinary healthcare teams
  • Mentor and supervise Physical Therapy Assistants
  • Maintain detailed clinical documentation and outcomes tracking
  • Ensure compliance with regulatory requirements and best practices

Required Qualifications

  • Doctor of Physical Therapy (DPT) from an accredited program
  • Current New York State Physical Therapy License
  • 2+ years of experience in skilled nursing or rehabilitation settings
  • Strong clinical reasoning and problem-solving abilities
  • Excellence in interpersonal communication and patient education
  • Proficiency in electronic medical records systems
