Occupational Therapist – SNF

Location : Dallas, OR

Schedule : Full-time

Setting : Skilled Nursing Facility (SNF)

Overview :

We’re seeking a compassionate and skilled Occupational Therapist (OT) to join a skilled nursing facility team in Dallas, OR . This role provides the chance to support residents in regaining independence and improving their quality of life through personalized therapy.

Responsibilities :

Conduct occupational therapy evaluations and deliver treatments in the skilled nursing setting

Develop individualized care plans tailored to patient goals and needs

Collaborate with interdisciplinary team members to ensure coordinated care

Maintain accurate, timely documentation per state and facility standards

Educate patients and families on therapy goals and home strategies to promote independence

Qualifications :

Active Oregon Occupational Therapist license

Experience in skilled nursing or geriatric care preferred

Strong clinical, communication, and organizational skills

Ability to work independently and within a team environment
