This full-time opportunity is ideal for a Board Certified Family Medicine or Internal Medicine physician with Fellowship training or a Certificate of Added Qualifications in Geriatric Medicine. As a Medical Director, you will lead a Program of All-Inclusive Care for the Elderly (PACE), making a profound impact on the lives of older adults and their communities.

Job Responsibilities :

• Supervise and provide general medical direction for participant care.
• Ensure delivery of care, clinical outcomes, and oversights of the Quality Assurance Performance Improvement (QAPI) program.
• Develop and maintain medical policies, procedures, and protocols.
• Lead and guide related Primary Care Providers (PCPs) and Advanced Practice Providers (APPs).
• Conduct assessments and develop medical treatment plans for new participants.
• Educate families on health maintenance and chronic medical conditions.
• Collaborate with the interdisciplinary team, participating in team and family conferences.
• Maintain physical examinations and manage chronic disease treatment for frail older adults.
• Participate in provider on-call rotation as required.
• Perform duties with excellent organizational, flexibility, and problem-solving skills.
