Your Role : Make an Impact!

As a Home Health Speech Language Pathologist , you will :

• Evaluate and provide tailored speech and language therapy services to patients in their homes or facilities.
• Collaborate on the development and periodic review of the Plan of Treatment and Plan of Care.
• Utilize your professional skills to assess and treat disorders related to speech, voice, language, hearing, and swallowing.
• Administer and interpret diagnostic tests, including audiologic screenings.
• Monitor and report changes in patients' conditions and responses to treatment.
• Train patients in alternative communication systems when appropriate.
• Provide guidance and support to patients, families, and healthcare staff.
• Maintain accurate documentation and timely reports as required.
• Participate in care coordination activities and discharge planning.
• Uphold the highest standards of confidentiality and professional conduct.
• Engage in ongoing professional development and continuing education.
