JOB DESCRIPTION : PHYSICAL THERAPIST
IMMEDIATE SUPERVISOR : Director of Physical Therapy
DUTIES
The Physical Therapist is a professional health worker who performs the following tasks :
1) Evaluates the patient by using specific tests and measurements to aid in determining the patient's musculoskeletal, neurological, cardiorespiratory, vascular, and functional status.
2) Plans a treatment program and sets goals for the patient as a result of the evaluation.
3) Treats the patient by utilizing exercises, physical agents, assistive devices, supportive appliances and other therapeutic procedures for the purposes of
4) Applies psychological and sociological principles in determining and carrying out the evaluation and treatment programs.
5) Functions in the prevention of disabilities.
6) Communicates and cooperates with other health and community workers in the total rehabilitation of the patient and in preventative health efforts.
7) Plans and administrates overall operation of physical therapy services in the facility.
8) Instructs and supervises supportive staff, patients, and family in carrying out specific procedures.
9) Assumes responsibility for preparing the required documentation whether written or electronic in a timely manner including the initial assessment, progress notes, discharge assessment, verbal orders, Medicare / Medicaid / Insurance Certification / Pre-Certification, and progress reports to the physicians as well as any required departmental forms.
10) Assumes duties and responsibilities as delegated by the Director of Rehab Services.
