The Physician Practice Coder II is responsible for throuroughly reviewing clinical documentation as appropriate to extract data and apply ICD-10 CM, ICD-CM / PCS, CPT and HCPCS codes as appropriate for billing, internal and external reporting, research and regulatory compliance. Assigns codes according to the ICD-10-CM, ICD-10 CM / PCS, CPT Official Guidelines for Coding and Reporting while ensuring the highest level of confidentiality, accuracy and timeliness. Requests additional information from physicians as needed to assign accurate codes. Abstracts and / or verify clinical documentation to ensure accuracy and support of diagnoses, procedures, charges, etc. Utilize resources and work collaboratively with team members as appropriate to ensure coding is completed within 5 days after discharge. Updates worklist daily to reflect unbilled reason for accounts not coded. Keeps manager and director informed of unusual concerns, and / or issues. Demonstrate accountability for accurate and timely completion of daily work assignments by meeting or exceeding daily productivity expectations. Must maintain acceptable coding accuracy / quality and productivity standards. Work with the Revenue Cycle Management to identify and resolve issues related to coding / billing per workflow. Collaborate with billing department to ensure all billing edits / delays are satisfied in a timely manner.

Requirements : High School diploma or equivalent and completion of a Coding program. Coding certification (CPC, CCS-P, AHIMA, and / or AAPC) required. LPN, RHIT, or RHIA desired. Requires two years medical office / physician practices coding experience. Prefer more than 2 years coding experience.
