Our client is seeking a dedicated and compassionate Occupational Therapist to join their Home Health team in Elizabethville, PA. The Occupational Therapist will provide in-home therapy services to patients as prescribed by their attending physician, while collaborating with the care team to enhance patient independence and overall quality of life.

Key Responsibilities

Conduct Occupational Therapy assessments to determine patient functional levels and develop individualized treatment plans focused on improving and restoring independence and physical / cognitive function

Implement and adjust treatment programs as approved by the attending physician

Educate patients and family members on therapeutic activities, adaptive strategies, and self-care procedures

Train patients and families on the use of orthotic, prosthetic, and adaptive devices; fabricate or modify equipment as needed

Reassess patient progress and update care plans accordingly

Maintain accurate documentation and submit reports in accordance with agency policy

Communicate with physicians and interdisciplinary team members regarding patient progress and recommended changes to care

Participate in agency meetings, in-service training, and case conferences

Maintain and enhance professional knowledge through ongoing education and development

Schedule and Shift Details

Full-time, Monday Friday, 8am 5pm (30 units per week)

No holidays, weekends, or on-call responsibilities

Travel

Local travel required within the service area

Required Qualifications

Bachelor's degree or higher in Occupational Therapy

1+ year of clinical experience

Valid Occupational Therapist, Registered (OTR) license in Pennsylvania (PA)

American Heart Association Basic Life Support (AHA BLS) certification (ACLS or PALS accepted)

Valid driver's license and reliable transportation

Experience with EMR systems preferred
