Position Summary

We are seeking a Home Health & Hospice Medical Social Worker (MSW) to join our caring and dedicated team. This is a Paid Per Visit position with flexible scheduling and a supportive environment that values compassionate, patient-centered care.The MSW will assess the psychosocial needs of patients and families, develop care plans, provide counseling, assist with resource coordination, and participate in the interdisciplinary team process.

Responsibilities

  • Conduct psychosocial assessments for home health & hospice patients and families.
  • Develop and implement individualized care plans to meet emotional, social, and financial needs.
  • Provide counseling and crisis intervention to patients and caregivers.
  • Educate families on available community resources, including financial, legal, and bereavement support.
  • Collaborate closely with the interdisciplinary team to ensure holistic patient care.
  • Participate in IDG (Interdisciplinary Group) meetings and care plan reviews.
  • Document visits and interventions accurately and timely in the EMR system.

Qualifications

  • Masters degree in social work (MSW) from an accredited school of social work.
  • Current state licensure (LMSW) required.
  • Minimum of one (1) year of social work experience in hospice, home health, or healthcare preferred.
  • Strong communication and organizational skills.
  • Ability to work independently and as part of a team.
  • Valid drivers license, auto insurance, and reliable transportation required.
