The Center provides long-term skilled nursing care and short-term post-acute rehabilitation, including a 30-bed ventilator unit. Job Summary :

The Occupational Therapist (OT) in a Skilled Nursing Facility is responsible for evaluating, planning, and implementing individualized therapeutic programs to help residents improve or maintain their ability to perform daily living activities. The OT works as part of an interdisciplinary team to promote patient independence, restore function, and prevent further disability in a supportive, resident-centered environment.

Job Duties :

Evaluate residents’ physical, cognitive, and psychosocial abilities using standardized assessments.

Develop and implement individualized treatment plans based on evaluation findings and functional goals.

Provide therapeutic interventions to improve ADLs (Activities of Daily Living), mobility, coordination, and upper extremity function.

Educate residents, families, and caregivers on adaptive techniques and use of assistive devices.

Recommend modifications to residents' environments to improve safety and independence.

Maintain accurate and timely documentation in compliance with federal, state, and facility regulations.

Collaborate with interdisciplinary team members, including nursing staff, physical therapists, and speech-language pathologists, to coordinate care.

Supervise Certified Occupational Therapy Assistants (COTAs) and ensure adherence to treatment plans.

Participate in care planning meetings and contribute to discharge planning processes.

Ensure compliance with SNF-specific therapy regulations and Medicare / Medicaid guidelines.

Promote resident dignity and uphold facility standards for quality care and ethical practice.

Education Requirements

• Master’s degree or Doctorate (OTD) in Occupational Therapy from an ACOTE-accredited program
• Successful completion of clinical fieldwork as part of the degree program
• Current state licensure (or eligibility) as an Occupational Therapist
• NBCOT (National Board for Certification in Occupational Therapy) certification
• CPR certification required.
