Job Overview\n\nFairview Health Services is searching for a hardworking RN for Speciality Infusion in Edina, Minnesota.\n\nThe Registered Nurse is responsible for the assessment, planning, delivery, and evaluation of nursing care for all assigned patient population(s). The RN is responsible for performing these responsibilities in accordance with all policy, procedures, professional practice guidelines and the MN Nurse Practice Act. The RN coordinates care with members of the health care team and delegates responsibilities to others as appropriate.\n\nWe believe the schedule will be between 7 : 00 am to 5 : 30 pm with every 6th weekend rotation.\n\nResponsibilities\n

  • Assesses patient needs and identifies expected nursing outcomes.\n
  • Involves the patient, family and other health team members in gathering data.\n
  • Uses an ongoing, systematic process to gather relevant information (biological, psychological, spiritual and social aspects) from patient and family.\n
  • Analyzes data and information to identify potential risks, prioritize needs and determine priority problems.\n
  • Identifies outcomes that are age appropriate, culturally appropriate, realistic and attainable.\n
  • Validates these with patient, family and other health team members.\n
  • Documents data and identified outcomes.\n
  • Develops an individualized nursing plan of care.\n
  • Collaborates with patient, family and other health team members to develop a nursing plan as part of an integrated, comprehensive plan of care\n
  • Develops an individualized plan based on age, social and cultural considerations, condition and other relevant needs.\n
  • Determines nursing interventions based on identified needs.\n
  • Identifies interdisciplinary resources needed to assist in implementing the nursing plan of care.\n
  • Prioritizes interventions based on patient acuity, available resources, and patient / family preferences and needs.\n
  • Documents and updates the plan of care to assure continuity throughout the continuum of care\n
  • Implements nursing interventions based on the plan of care.\n
  • Provides care according to the plan, and in accordance with all relevant policies, procedures and nursing practice guidelines.\n
  • Coordinates nursing care with the care of all other health team members.\n
  • Initiates age and culturally appropriate measures to support and meet patient needs while achieving expected outcomes.\n
  • Initiates patient / family teaching as appropriate to age, readiness to learn and available resources.\n
  • Collaborates, consults, or makes referrals if unable to perform safely and independently.\n
  • Delegates direct and indirect patient care activities to appropriate personnel based on needs, condition, potential for risk / harm, complexity of the skill and predictability of the outcome.\n
  • Assures all actions are based on scientific, ethical or ecological principles; proven effectiveness; past experience; practice standards; patient / family requests and values; or, legal parameters.\n
  • Uses the full range of therapeutic modalities to support and augment the healing process, moving from least intensive and invasive to more, depending on need and context.\n
  • Documents interventions according to practice guidelines.\n
  • Anticipates, recognizes and responds to emergent situations.\n
  • Evaluates care and patient response to interventions and expected outcomes.\n
  • Systematically and continuously evaluates patient's response to interventions and the effectiveness of the plan of care\n
  • Modifies the interventions based on patient response and changing patient condition.\n
  • Involves the patient, family and other health team members in evaluating care.\n
  • Uses ongoing assessment data to revise patient needs; priority problems, nursing outcomes and the plan of care.\n
  • Uses communication strategies to achieve desirable outcomes.\n
  • Communicates verbal and written information in a clear, concise, accurate, and timely manner.\n
  • Uses communication strategies appropriate to patient / family needs for information, education, and support.\n
  • Collaborates and communicates effectively and respectfully with the patient, family and members of the health care team within and across care settings to promote partnership-based health care\n
  • Demonstrates the ability to advance care by utilizing standards, guidelines, care pathways and continuous improvement to enhance patient outcomes.\n
  • Identifies gaps in care and practice and brings them forward to applicable person(s)\n
  • Utilizes improvement methodologies\n
  • Incorporates data and information into practice to continuously improve or innovate care\n
  • Promotes a culture of evidence based practice (EBP) by participation in any of, but not limited to, the following activities : \n
  • Utilizes evidence based policies in clinical practice\n
  • Attends and participate in learning opportunities about EBP\n
  • Member of hospital or area based nursing practice council or research council\n
  • Completes an evidence based practice project (e.g. policy development or poster presentation)\n
  • Presents an EBP finding to your peers\n
  • Journal club member\n
  • Critiques and circulate a journal article relating to nursing practice\n
  • Reviews the literature on a practice question\n
  • Develops a standard / pathway / guideline for patient care\n
  • Participates in a research study\n
  • Demonstrates ability to provide care or service adjusting approaches to reflect developmental level and cultural differences of population served.\n
  • Information Technology : \n
  • Basic Computer Competencies – using the computer, email, basic use of word / excel, and web browsing\n
  • Information Literacy – ability to determine information needed, efficiently access and critically evaluates that information, apply use information for patient care, and evaluate the use of information.\n
  • Information Management – verbalize importance of health information systems for nursing practice, maintain confidentiality, privacy and security of protected patient health information, adequately navigate health information systems, and abide by policies and procedures pertinent to health information system use.\n\nRequired\n\nExperience\n
  • Required : 1-2 years of chemotherapy administration and / or other infusion experience with competencies in IV insertions such as central lines, PICC, Ports, Peripheral IV placement and infusion devices. Experience in administering antibiotics, monoclonal antibodies, chemotherapy, blood products and associated medications.\n
  • Specific Adult or Pediatric experienced based on population served.\n\nLicense / Certification / Registration\n
  • Current Oncology Certified Nurse Certification or ability to obtain OCN certification within 1-2 years of date of hire based on FTE appointment.\n
  • Valid RN MN License\n
  • MN Registered Nurse (RN) License\n
  • Basic Life Support (American Heart Assoc or Red Cross)\n\nPreffered\n
