Responsibilities :

As the Director of Case Management, you will lead and oversee our discharge team to ensure efficient and effective patient transitions and care coordination. Your responsibilities will include :

• Providing strategic direction and leadership to the case management department.
• Collaborating with interdisciplinary teams to develop and implement patient-centered care plans.
• Managing utilization review processes and ensuring compliance with regulatory standards.
• Identifying and implementing process improvements to enhance patient care and outcomes.
• Mentoring and developing case management staff to achieve excellence in their roles.
• Utilizing data and analytics to drive performance improvements and cost-effective care.
