HumanEdge Health is seeking an experienced

Travel RN Case Manager

for an acute care hospital in

Jackson, Mississippi

• . The ideal candidate will have a strong background in

acute care discharge planning

and

case management

within a

Level 1 trauma or large academic medical center

This is a

day shift position (Monday–Friday, 8 : 00 a.m.–4 : 30 p.m.)

with

no weekends or on-call

required.

Responsibilities : Perform

acute care case management

focused on

discharge planning

Manage a

caseload of 15–20 patients per day

across inpatient units

Collaborate with

social workers

physicians

nursing

pharmacy

, and

RT

for discharge readiness

Utilize

naviHealth

for referrals and discharge coordination (transitioning to

Epic Care Management

Assist in safe transitions of care, including medical and social discharges

Work in a

Level 1 trauma center

serving a diverse patient population

Report directly to the

Social Work Manager

and collaborate with the

Chief of Care Coordination

Requirements : Active RN license

(Compact or MS)

2+ years of acute care case management experience

in discharge planning

At least 6 months

in a large academic or Level 1 trauma hospital

BLS certification (Required)

EPIC charting experience (Required)

CCM certification preferred

(not required)

BSN preferred

(ADN considered)

Flu vaccine required

(medical exemption only with documentation)

Epic Systems Corporation – EpicCare Inpatient (EMR 2014 Certified EHR Suite)
